Here's a copy and paste from a dive into Lilly's data concerning Reta and heart rate increase:

The TRIUMPH-4 Phase 3 results (topline data released in December 2025 ) confirmed that retatrutide—the "triple agonist"—is arguably the most potent weight-loss medication to date, but the heart rate increase remains a key physiological signature of its glucagon component.

Here is the breakdown of the heart rate data and the broader clinical context from the trial.

1. Heart Rate Increase Summary​[archived internal link]
In the TRIUMPH-4 study (which followed over 400 adults for 68 weeks), the heart rate trends were consistent with what was seen in Phase 2. While the exact final "bpm" average for TRIUMPH-4 will be fully detailed in the upcoming peer-reviewed publication, the topline trends are:

Dose-Dependent Increase: There was a clear increase in resting heart rate (RHR) that scaled with the dose (9mg and 12mg).

The "Peak and Decline" Pattern: Similar to Phase 2, the heart rate increase in Phase 3 typically peaked around Week 24 and then saw a gradual decline toward the end of the 68-week trial.

Magnitude: Based on the cumulative data, retatrutide's heart rate increase is slightly higher than that of Ozempic or Mounjaro (which typically see a 2–4 bpm increase). Retatrutide has shown a mean peak increase of approximately 7–10 beats per minute (bpm) .

Glucagon’s Role: Unlike GLP-1/GIP drugs, retatrutide’s third target—the glucagon receptor —is believed to be the driver of this higher heart rate, as glucagon has known chronotropic (heart rate increasing) effects.

2. The "Cardiovascular Paradox"​[archived internal link]
The most interesting part of the TRIUMPH-4 data to be the relationship between the heart rate and blood pressure. Despite the increase in heart rate, the medication showed massive cardiovascular risk reductions :

Blood Pressure: Participants on the 12mg dose saw a systolic blood pressure (SBP) reduction of 14.0 mmHg [3.2].

Lipid Profile: Significant reductions were seen in non-HDL cholesterol, triglycerides, and hsCRP (a marker of systemic inflammation).

Clinical Takeaway: The trial suggests that the profound metabolic benefits—massive weight loss and systemic inflammation reduction—appear to "outweigh" the physiological signal of an increased heart rate, though long-term outcomes are still being monitored in the ongoing TRIUMPH-Outcomes trial (expected 2029).
 
I just started Reta two weeks ago. I started at 4mg because I was already on Tirz at 10mg. I am going to titrate my Tirz down to 7.5mg and keep Reta at 4mg for next two weeks. Pinning Reta on Friday and Tirz on Monday.
 
Currently on 4mg Reta (4 weeks) after 3mo on a tirz cycle (2.5/5/7.5) and Reta has been better. Had some fatigue on tirz and the normal GI stuff, but Reta has been much better in those regards. I didn’t titrate down on tirz and haven’t had any ill effects.

Also, body recomp experience has been better compared to tirz, and I’m attributing that to the glucagon receptor component. One thing I’d say is staying hydrated is key for myself.

I’m going to titrate up to 5mg for the 2nd month with the plan to titrate the same as tirz. But I understand why people stay at certain mg for extended periods because of efficacy. I’ll see how 5mg goes, and if it’s still rocking then might just stay there. Potentially adding CJC/IPA and Klow cycles in near future.

I’m taking the low and slow approach.
 
Just mixed two vials of a new batch last night. We did 6mg this morning, first 6. This is the 11th pin since we started 1/9.
 
Today was my second 6mg 2x weekly dose. I started Tirz on 12/12/25 and started the bridge to Reta on 1/10/26. I've taken a few Tirz shots after 1/10. In total 13 doses of Tirz, 29 of Reta.
 
so I have Hashimoto, and I am suspecting that it has been getting worse because no amount of synthroid got my TSH below 2.0. Only after I added T3 it stabilize after 6 yrs of miserable experience.

I also got super hungry on with T3...I guess it restored my metabolism to where it was decade ago? But taking reta really dialed back the hunger
 
Been on reta for 7 months (shifted to reta from 5 months of tirz before that). Started at 0.75mg/week, now at 0.25mg/week. I was losing a little weight (down 5 pounds), so I backed off. I take glps for prediabetes and A1c is 5.0 (down from a high of 6.1). I respond very well to these drugs.
 
What is the absolute lowest dose people take? I was taking 2-5 mg for only February (never took any glp-1 previously). Last dose I had was 2.5 mg about three weeks ago. Am thinking of taking only 0.25 mg per week now. Maybe 0.5 or 1mg. Or every ten or fifteen days. Like just using it as a tool for when my diet is sliding away from perfection. Cost is not an issue. It is very cheap.

Any thoughts?

I don't understand how those low doses work, but I guess they do. Most do weekly or biweekly pins due to the half life of Reta which is 6 days I read.
 
I went 2mg... 4mg.... 5mg.... 6mg... 8mg... All these were 1 weeks apart. I think I will stick with 8mg and do it every 5 days and add Cagri. I was previously on Tirze.

How do you like the blend like that? Do you end up with no appetite?

I know they say start low and go slow, but I came from Tirz high dose. I titrated up over 8 weeks, then I was on 8 mg for 2 months, now down to 6 mg the last 3 weeks, because I got anhedone.

I'm very close to my goal weight, so it's slow weight loss now.

I had been at 15 mg tirz and was ready for maintenance but then my weight started creeping up. I gained about 12 lbs. I stopped the tirz for three weeks and then restarted at 10 mg/week. I’d now like to add Reta but don’t know if I should start at 1 mg/5-6 days or more than that. I wondered what your experience was/is? Thank you for sharing this!
 
Just finished the 10th week at 2mg.

Started at 208, down to 182 (26lbs lost!).

Goal is 175lbs.

Love the feeling.

I've been in calorie deficit this whole time but haven't felt the common effects of, well, starving.

I eat whole macros and do the protein shakes/powder, take micros, hit the water requirements, electrolytes and exercise.

Love the "Quiet Mind" of Reta.

I had one sweet in 10 weeks: a wonderful pastry at a historic bakery one week ago.

I used to be a chronic ice cream eater.

I was gonna get to 1mg and try to get caloric neutral while keeping metabolism sped up. Anyone got out of deficit and stayed in the Reta?
 
Just hopped back on Reta at 1mg 2x/wk. Let the fun commence! Now to drop calories back down 🙂

In general I “only” take Reta. Since I started taking Reta on 1/10 I’ve had 6 pins of Tirz totaling 14mg, 98mg of Reta in 30 pins. Down from 272lb to 242lb in that time.

How long did you take 10 mg then ?

Try 1 mg every 5-6 days is probably a good idea, and if you are like me, go up one mg per week. Just know that it's not recommended. Because of how it works and because of possible side effects. Go down in Tirz at the same time.

I have great experience, no side effects, until after a few months on 8 mg reta, I had some anhedonia show up again, so went down to 6 mg.

I am one of those who are on the aggressive side and not afraid of raising my doses quickly, and i don't mind going above the recommended dosage because i think i'm one of the rarer kinds that respond well to the medication, but also build tolerance quite quickly. So the best strategy for me is to go hard and fast while remain somewhat healthy at the same time.

I went from 15 mg of tirz and quickly titrated up to to 10 mg reta. It's been great.
 
Just started Reta this week, 1mg once a week for first 4 weeks was the recommendation made to me. Will move to 2mg on week 5.
