Not Good - Gaining on 15 mg Tirz

I have done every diet, even had RNY, but Tirz nailed the weightloss for me. BUT. The last 6 months I’ve seen a slow increase in my weight despite no change in exercise or calorie intake. Like about 10 pounds over that period (hey, that is slow for me). I think the food noise has returned a bit too. I’m still at 15 mg/week, pinned once a week. I’ve posted about whether I should add Reta or Cagri before but I wanted to take one more shot at getting your all’s opinion or experience on what worked for you. Or do I add sema? I’m pretty confused about the best path forward.

What has your experience been? I’m taking action on this TODAY! I can’t stand this. I can’t bear to go back where I came from.

Did you make it to goal before you started gaining?

Many people here dose tirz above 15mg. I stalled before goal while taking tirz, so I slowly stacked with reta and broke my plateau. For me, it was a gradual process of adding reta while pulling back on tirz. I took about 2 months to switch.
 
I got below goal without breaking a sweat (meaning without mentally obsessing over it). Honestly, got too lean. Looked like I was older than I am and I am old - so that’s saying something. Honestly, I have been surprised at the following gain so I think it’s taken me awhile to react - until my love handles started spilling over my jeans (again - gah!!).
 
Similar stage and my weight loss has been in dribbles over the last few months. 2 weeks ago I upped to 15mg, hoping for a boost, but nothing noticeable at all. I was jabbing every 6 days.

This past week I decided to mix things up a bit....dropped my dose back to 12.5 but jabbed at day 5, not 6, moving to a 5 day rotation. At 12.5 every 5 days vs 15 every 6 days it should essentially maintain same med level, six of one, etc.

But, to my utter shock, I'm suddenly dropping weight DAILY, 3 pounds in 3 days! I have no explanation for this. I did, at the same time, add an electrolyte to my life, but that's the only other thing that changed.

I can't explain this, just putting it out there for you as a data point. 🤷

I had RNY in 2003 and tirzepatide has really nailed the weightloss for me too. I’m currently on 17mg, but that’s down from 20mg not so recently. But I’m also stacking now with elora. I previously stacked with sema and cagri, for brief periods.

Since you’ve reached your goal, it sounds like more of a maintenance issue. There really isn’t a lot of clinical guidance in this area or high-quality research guiding us on the best ways to maintain. So, most people are taking ad-hoc approaches and throwing spaghetti at the wall and seeing what sticks.

If I were you, I’d try stacking with a little bit of sema or cagri rather than increasing above 15mg of tirzepatide. You might also need to dial in your diet for a few months to lose those 10lbs. Although you say nothing has changed in your diet, I’m wondering if there could be changes. The only reason I say not to increase the tirzepatide is that I now think that there are diminishing returns on higher doses and that higher doses probably just make sides worse. At least that's what I'm blaming (currently) on my trashy sleep. I’m trying to get back down to 15mg.

first of all, I am very sorry you are experiencing this, I know that it is probably causing a lot of anxiety because you dont want to ever go back to where you came from. six months is a pretty long time, but those pounds can creep on slowly. That's how I gained, and didnt really see it happening. So, I think after you lose weight, your body not only has a muscle memory, but also a fat memory and it will quickly betray you if you even slip up slightly! That is cruel, but luckily, we do have some tools to help! I think this could be a perfect case for adding Reta because that raises metabolic rate a bit? Also, have you been stressed? Cortisol changes? Sleeping ok? If I wanted to take action TODAY: I think id commit to a week of very clean eating and see if adding reta would give you a boost in the right direction again. Dont beat yourself up over it tho, sounds like you are already making the commitment to rein it in. Good luck! You do have a road of options in front of you.

1. Take more Tirz. They are currently testing 20-25 mg. Should have results at EOY.

2. Stack some Elora (1- 2 mg). This seems to be a better alternative to Cag at this stage.

3. Fine-tune a supplemental peptide cycle mitochondria specifically ( SS31, MOTS-c, NAD+)

Expect slight water retention from the next two. Which usually resolves.

4. Secretagogues ( Using your own GH): Tesa, CJC-1295 no DAC with IPA)

5. HGH- Adding growth Hormone

6. Lift more weights at least 3 x a week. Also do cardio fasted for lipolysis.

7. Fine-tune diet

8. Add 5-Amino-1mq and AOD-9604. These make very subtle differences but a difference nonetheless.

The only thing I'm not sure about is #2. Cagri seems to work really well for some folks with little side effects, and with the elora supply so wonky right now, I'd personally recommend that folks try cagri first to see if they can tolerate it and if it's effective. I'd still be using it cagri if not for the fatigue. It's less expensive, has a longer track record, and testing is reasonable. I would just say not to make the same mistake that I made buying a bunch of cheap cagri kits, just to find out that it wasn't for me.
 
Your caloric needs are less, that is why no change in calories is producing weight gain instead of maintenance.

So you have to reduce your calories. Or increase your workouts without increasing your calories.

I was thinking of you with the Cag. I can't do exhaustion. My schedule is not accommodating. You are the reason I never tried it when I first came here. Lol.
 
Did you start a different batch of tirz around the time the weight gain started?

I did do a lot of whining about the fatigue with cagri 🤣 !

Given the choice, good quality elora is probably a better choice than cagri. I'm a practical guy and I was kind of factoring the current elora marketplace. What I'm seeing is a bunch of vendors saying they have it, but no testing to support that it's really elora or that it's structure is correct. I've seen a COA saying that a 5mg vial tested .5mg.

That just leaves the gorilla in the room. I'm okay with the gorilla's pricing and probably never would have started cagri if I knew that elora was so close to being available in the grey market. So, you're right about #2 as long as OP is okay with the gorilla. 🤣

This would be my first question too.

And fwiw, I have been at the same weight for almost a year at this point, but it is up about 13 pounds from my lowest. That seems to be the 'no effort' zone, at least for me.

I'm snail crawling up a reta titration stack with my tirz, just bought some Elora yesterday, and beginning a new round of actual effort. For me, it doesn't seem to matter if I take 20mg, 15mg, or 12.5mg tirz - I get the same feels.
 
Hate to be that guy, but are you sure your calories haven't changed? Very rare to start gaining on same calories. Tirz doesn't burn calories, sure some metabolic stuff, but mostly appetite suppression. In maintenance mode, its easy for one or two small additional food items to start stacking calories.

First thing I would do in your case, is keep dose same of tirz and try every 6 days or 5 days as many see the hunger return, a day or two before their next shot.
 
Off topic, but OP @When_Pigs_Fly lives in one of my favorite places to visit. Wife and I usually go once a year. We have fun there, about 4-5 hours away depending on traffic, season or which way I go.

100% this. Should always have a variety of vendors. The cream will rise to the top. Between compound and grey I have approximately 11 different types. Most are great and a few not so much. They go to the bottom of the stash and most likely will never be used again. Thankfully single vials are my weakest. Kits are all great.
 
Yeah with food noise returning, but not being in any sort of huge crisis (I know "it's just 10 lbs" might sound trite - it's not meant to be! You've done a great job so far - I'm jealous of anyone who hit goal and is working on the maintenance side now) you do have quite a few options here.

Maybe the first one to try is just to change your injection schedule so you're injecting 15mg every 5 days instead of 7.

Obviously that bumps up the total dose effectively, but not by a ton and it might turn the noise down in days 5,6 and 7 post-shot

Don't hate, that's almost assuredly the issue. Some combination of more calories in or less calories out. The fix is to eat less or move more. To eat less, use more medicine, more tracking/conscious effort, or a combination.

That's a 40% increase in effective weekly dose. That's 15mg to 21mg.

First, I want to say I appreciate that you're one of the rare posters on here who understands that a stall is to be expected, while regain is the actual indicator that something isn't working right for you.

I'd give actionable steps, but @chewonmysac gave you the correct answer so there would be no point in me posting the same thing that was already shared.

The only thing I'd add to that, is that it might be useful to picture weight loss/maintenance as two teams engaged in a game of tug of war and each player representing different factors. That's to say that just because you're taking a GLP, there's no rule that you can't simultaneously engage in different dietary strategies to see what the cumulative effect might be for your body and that while chew's list is great, past non-peptide strategies that led to weight loss for you could be worth revisiting too.
