Gastric Sleeve and Reta

I got a sleeve done 10 years ago. It got me to a place I'm ok with, but I am hoping that Reta will help in my goal to reach a BMI under 30. In my mind it seems like a really obvious good pairing. Anyone have experience with this?
 
Assuming the stomach has now stretched since and is back to being normal? I wouldn't see an issue, minimum side effects are not stomach related if you have any, its quite common for people to have recent surgery and be on Mounjaro, Reta would be the same 🙂

It has stretched, but I can still feel restricted if I follow the high protein, low carb rules that my doctor put me on, and I can lose weight. The hardest part though is what happens when I slip up after a few months and then binge on carbs. I tried Tirz through a family member, and even at a very low trial dose, I was able to slip right into good habits due to the food noise reduction. I went up to the normal starting dose and I felt like I couldn't eat anything, so I stopped. When I heard that Reta suppresses food noise but doesn't suppress appetite as much, I thought it would be just perfect.
 
Most people make the mistake of assuming gastric surgeries lead to weight loss purely due to a reduction in stomach size. Although that does play a role, it's not the full story. Part of the surgery usually involves severing at least part of the vagus nerve, which relays satiety hormone (e.g. GLP-1) signals from the gut to the brain (so-called afferent signaling). That reduction or elimination in afferent signaling plays a major role in weight loss. This has been confirmed in rodents by performing surgeries on them where the stomach itself is left alone and just the nerve is severed leading to significant weight loss.

There was another user on this forum who found that meant (at least for them) that the overall effect of gastric surgery plus GLP was cumulative with them losing significantly more weight on a GLP after surgery-related loss had stalled. I wouldn't assume you'll get the same result they did, but we can certainly cross our fingers and hope that ends up being the outcome for you.
 
Reta or Tirz would be fine. You don’t have to dose Tirz to the point of food aversion. If you’re not going to get highly enough on Reta for the bmr boost, Tirz is probably just as good.

How much weight are you looking to lose?

Like Tubby said, I’d expect some level of unique response due to the changes in ghrelin and glp production.

I was 370 at my highest. I had surgery at 340. I went down to 260 after surgery, and I've yo-yoed between 280 and 305 for the last 5 years or so. When I did tirz for 3 weeks, I lost 14 lbs. in that time. I'm 6'3". I would like to be under 240, putting me out of morbid obesity. So 235 would be a great goal for me. I have a lot of muscle, and I'll be working very hard to lose as little muscle as possible.

Nah, the stomach doesn't stretch just because time has passed. It stretches when a person consistently overfills themselves. I had my sleeve done over 10 years ago and I still can't eat a large volume of food.

Sounds like you’ve got a bit to lose. I’m am inch or two shorter, started Tirz 16.5 weeks ago at 282lb and was 238lb this morning. I switched to Reta for a few reasons but the theory of lean mass sparing was one.

I’d recommend a DEXA scan to start off with and periodically to see how your preservation efforts are doing.

I’m doing Reta, trt, and hmb-fa to reduce catabolic losses. Lost 18.7lb between my last two scans, maybe half a pound of that was lean mass.

I had my VSG 17 years ago, I was almost 600lbs at my highest. Like you, it got me down to about 250lbs and I maintained that for 7-8 years. Then things started getting Carbalious, I ballooned back up to about 400lbs and needed my knees replaced. I did one, after I got down to 300lbs and just doing that on my own. Then I got back up to 325, after being off work for 3 months. I started on Compounded Tirz, I lost 125lbs in 16 months and gained 4lbs of lean mass in that time via 3 Dexa scans. The last 23lbs I lost was on Reta, that was in one month on 4mg. Lost about 400lbs total, I take 1mg of Tirz & 1mg of Reta and alternate shots every 84 hours for maintenance and got my BMI from 84 to 27. Mind you I didn't work out in this time, I did have lots and lots of Physical Therapy though. Diet was good, did some Lipo C & NAD+ at the time as well.

Reta is good stuff, find a good source, start low and slow and work your way into it. It can jack up your resting heart rate, supplement with Taurine if it does, certainly doesn't have the food suppression Tirz does and stay on top of your protein and electrolyte game.
 
I am now a week in from starting and want to give a little update. I went ahead and decided to do two 1mg doses for my first week, and tomorrow I will start week 2 with a single 2mg dose. My plan is to maintain a 2mg dose for 4 weeks, almost certainly go up to 4mg, and then have no plan to increase unless needed. (I've gotten a lot of good advice about titrating up as a last option rather than a first).

That said, wow. I really like this. I have had 0% food aversion, which was the worst feeling on tirz. I can think about food and eat with no problem so far, but the feeling of satisfaction when I eat is amazing. I might be imagining it, but I feel somehow warmer internally, sort of like the feeling of being in ketosis, but I am not doing low carb, only high protein. I have eaten what feels to me like a lot, but I'm already dropping, 3lbs the first week.

I really appreciate that I can eat. I really did not like how tirz made me feel repulsed by food. At the same time, food noise is pretty much non-existent and I am eating what I believe to be a healthy amount of food and feeling lasting satisfaction from it.

Does this experience relate to anyone else's?

BTW, if you are asking why I already plan to go up in 4 weeks, it is for the purpose of getting to what the research has shown to be an effective dose for the 3rd glucagon receptor. If it is too much, I will certainly reduce. I am hoping that 4mg will be a long term dose for me.

Well I was very hungry on Reta until I got to 6.0. So I had to add tirz in for hunger purposes, I never followed protocol. Do both to this day and I am on 8 Reta.
 
My doctor wants me to consume about 2000 calories a day and take in 130g of protein while I lose weight. I'm on week 2 of 2mg, and I'm having a hard time consuming more than 1500 calories because I feel so full. I only sometimes meet the protein requirement. When I was on tirz, I went days where I couldn't take in 600 calories. That really wasn't healthy. I figure as my system gets used to it, I will be able to eat more, and I should be able to go up to 4mg.
 
you should stay on 2mg for 4 weeks

after 1 month the Reta will be 100% in effect

there is no need to go to 4mg if you cant eat at least 2000 cal daily

I didn’t have a sleeve, but a duodenal switch w/biliopancreatic diversion. It’s a sleeve w/a bypass. Tirz has been more effective, but I have sides on Tirz alone (crushing fatigue, food tasted strange, concrete constipation) so I stack w/Reta.

Yeah the only way I met my protein goals easily was by adding unflavored protein powder to my coffee (mix it w/cool/cold liquid first, then add in the hot coffee slowly or it clumps). I’d do 2-3 scoops with a cup of Darigold fit milk, an envelope of high protein carnation instant breakfast and 2 cups of coffee. (Around 107-120 gms of protein in one shake). I feel like I have to do this, because I’m not a breakfast person..
 
I had RNY 22 years ago. I never can eat a lot of food. I am restricted for life. But I stopped losing weight after the first year post op. A few pounds even came back. GLPs saved my life, I finally lost the final 40 pounds and got below my goal weight. It worked so well. Started on sema, then tirz, then Reta.

What is your tirz dose and how do you schedule your pins, if you don't mind my asking?

I am 5 years out in June, and have maintained really well, and really recomp'd my body well. Although like you -- food noise was LOUD, and that paired with perimenopause/menopause has caused me to fight with about 10lbs of ...regain or inflammation, no idea. I started with a super low Tirz dose last January and was able to lose all of that regain and hold steady, but the food aversions have started a little more lately. I just had an open shoulder in February that was a lot hairier than anticipated, so it's kept me out of the gym for two months, and I've only been able to do light cardio til recently when I started ramping that back up to pre-shoulder levels. Because I couldn't move and was still eating to protect my pouch from the meds for my shoulder, I am up those damn 10lbs again! Currently I am up to a split dose around 8mg of tirz a week and will be adding in a low dose of Reta here soon to see if I can get a handle on it all again. I've had a vial waiting for reconstitution for like a month that I've held off on mixing because I felt like it was a waste if I wasn't doing much exercise! Aiming to get that started hopefully this coming week.

I do an interesting dosing schedule. 1 mg tirz, two days later 3 mg Reta, two days later 1 mg tirz, two days later 3 mg Reta.
