Food noise again

So I have been doing Tirz since October of 2025 and the food noise was gone and I would get nauseous. I started slow and I am on 5mg dose and have been for 5 weeks. I take my dose on Saturdays. Today I am struggling with the noise. I think it is worse today than anyday since I started. I know I am not following the Eli recommendations but I really don't want to be at the high doses too quickly if I can get away with continuing on the lower end of things.

You're at a dose that's below typical titration schedules because you don't want to be at high doses if you can get away with lower doses but you're struggling with food noise...

I understand none of what you posted was a question, but you covered the whole problem and solution in 6 sentences.

Hi 33, couple of things - did you open a new vial? Sourcing the same, etc..? Thinking about sometimes from different sourcings, different potencies and all. Next - are you hitting your protein goals? About a gram per pound you want to weigh. Third - are you sleeping enough? Fourth - Drinking enough water and a bit of electrolytes? I started with Tirz and it was awesome. When I started with RETA, it was CRAP! It took weeks to titrate to about 3mg when the noise went away. I also added .25 to .50mg of Cagri a day after I do the Reta. Maybe Cagri - just a touch - would help, if everything else is checked off? Good luck. Hope something here helps.
 
If you can get away with lower than normal doses, that's great. But that doesn't seem to be happening here. I suggest titrating up before adding anything else.

Maybe add cagri?

I second / third this, as this is also basically what woundcarping said.

Having been on for 6 months and only at 5mg- you’re in absolutely no danger of hitting a “high dosage” anytime soon. 1-2mg could make all the difference for you and not require anything else to source or risk or work into your regimen

Don't have to follow EL plan. Maybe try 6mg and bump up if that's not enough
 
I found my food noise returning on 5mg in the middle of the week. I take my injection on Fridays now at 5mg and I added a 500mcg bump dose on Tuesday and that really seems to help just push my levels up a notch to tide me over. If I need more, I'll bump that Tuesday up to 1mg...this helps keep my levels more consistent instead of the spike then fall off of efficacy.
 
Titrating up is the obvious solution. Being at just 5mg after 6 months was bound to result in a reawakening of your appetite. But if you're really opposed to titrating up, I agree with those who suggest split-dosing and/or adding Cagrilintide.
 
I think the obvious answer is to increase the dose of tirz. If you have a lot of weight to lose trying to stay on low doses seems counterproductive. But if you really do not want to increase the dose yet, you can always do a top up dose of 1-2mg if extra hungry, most likely it will work, and then wait and see . If a week later the same thing happens after a dose of 5mg then you have a definite answer that increasing the dose is a good idea and will work. The odds of getting significant gut side effects are low - usually it is difficult to be really hungry and nauseous at the same time, and that is the most common side effect.

Without knowing anything about your age or start weight I cannot say for certain that being on tirzepatide is beneficial for long term health, but it is for most people and more so if older, sicker or more obese. So in general being on it at full doses is helpful rather than harmful to long term health, then staying on low doses in the absence of side effects, while being hungry does not have a lot going for it.

It was the last dose of the vial. I couldn't remember and I wondered that on the way home. NO I am not hitting my protein and I will say sometimes when I am struggling with hunger and I drink a protein drink it does make a difference. I have to incorporate some electrolytes as I don't use them. I started out using them but did not follow through. I have a hard time with that I have to drink protein drinks. It was a weekend so NO on the enough water. I am good during the week but not so much on the weekend. I never seem to get enough sleep even with a cpap machine. Thank you for your questions I will reevaluate how I am going about this. I don't think I need to go up fast in my mg I just need to pay more attention to what I am NOT doing. <3

Sorry I wasn't clear enough and asked a direct question. As you can tell I really am not a genius for finding the correct words.

Not really opposed to titrating up but want to go slower because what happens when I get to the top? Where else do I have to go to continue to lose weight? That might be a stupid response but I don't want to top out.

I am 58, start weight 210#, current weight 186#.

With how cautious you are, and how patient you've been with your titrations up, it's genuinely hard for me to see you hitting 15mg (a conservative "top" dose at this point) for another year.

What is your goal weight? and your height?

You must be really close to your goal! So I don't think you have anything to worry about with respect to topping out. But if you can get a hold of some Cagrilintide, that would definitely add a whole new dimension to your appetite suppression. It works on a different receptor--killing your hunger in a whole new way that your brain cells have not been desensitized to yet.

But you might just give titrating up on Tirz a chance so that you don't have to make a whole new purchase. Make sure you start walking at least 10,000 steps a day. That will nullify an extra 400 calories you might be tempted to eat.

I have never tried Cagri but Sema is the OG at controlling appetite. It’s dirt cheap too. I recently added it back to my regimen. It works very well and no issue at low dose. I’m taking 0.75 mg per week. I skipped the 0.25 mg starter dose, did 0.5 mg for two weeks then settled at 0.75mg. I’ve been taking GLP1’s for 2.5 years and originally took Sema only.

"Don't borrow trouble" as my grandmother would say. You're getting anxious about a non-existent "problem" that you are making up in your own head.

Increase your dose. Come back and post asking for help if you get to 15mg and you feel it's ineffective.

You can always swich to another or add to the stack.
