What is the strongest concentration that is ok for a vial of Tirzepatide?

I’m trying to help my friend out, but FFS my brain aint brainin’ today.

She has been on 10mg of Zepbound. She is a newbie greyzoner now. She would rather not have to inject a large dose each time, which I get. I got her pushed toward filtering, and sterile new vials.

Anyway what concentration is considered a danger zone for Tirz? 40mg/ml? 60mg/mL?

I think she is considering two 30mg vials with 1ml of BAC total. That would put her at 60mg/mL in her new vial. I’ve only gone as high as 30mg/mL. I even had a dose calculator scream at me when I fat-fingered and accidentally setting it higher than 30mg/mL.

My head is spinning. Help a hound out?

How much was her vial tested as? 64mg, 69mg (nice) or around there? Technically I've done 1mL of BAC for a 64.21mg vial and only did about 13u.

Try this calculator and input the amount that batch was tested at, not 60mg, then add 1mL BAC and you'll see a warning. Keep adding BAC to see when it goes to blue or green. I was fine with 1mL of BAC and less units.

If she has a 60 mg vial and she takes 10 mg. That’s 6 weeks per vial or you asking something else…
Something else. But it's still not 60.00mg in the vial due to overfill. But they're asking what a safe concentration is. I did 64.21mg/mL and it was fine.

If she is adding 1mL to each vial of 30mg and then filtering them both into one vial ... her new filtered vial will contain 60mg in 2mL of fluid. Since her desired dose is 10mg she would want each injection to be 33 units.

OR

If she is is only adding .5mL into each vial then she has a 60mg vial in 1mL of liquid and her desired dose of 10mg would be had in 17units.

Yeah that’s what I was asking. What was the safest concentration for someone to have once they reconstitute Tirz.

What the hell do people do with those 100mg vials that I’ve seen selling? Holy crap! lol

She did .5mL for each one and then was gonna combine those in a new vial after filtering.

If it dissolves then it should be okay…make sure there’s not much liquid left behind…those last drops will be concentrated…I don’t like worrying about how much API I may have left behind…

There are 120mg vials that are probably overfilled to 130 or maybe even 140mg. No idea how you'd use that unless you're sharing.

I've done 64.21mg/mL without any issues. Just depends on how many units they want to inject.

Overfill changes those calculations, so it's not going to just be 17 units. 🤷‍♂️

It's a 60mg vial. So every 10 units is 6mg.... but they're taking into consideration an over fill of 4 to 9mg which I don't worry about. If I'm pinning 15mg and it's actually 17.3mg or whatever I care not cuz I'll do the same in 5 or 6 days again.

I only mention it for people who aren't used to this, just starting out, etc. You're a seasoned veteran so it won't affect you. ❤️

I gave her two of my 30mg vials, to get her through the month. The over-fill avg for our group testing was 2.xx mg, which means she actually just reconstituted about 65 mg.

So 65mg/mL

Effing math. I’ve been smothered in IT work all day, so now I have the retention of a screen door.

Use that, it'll let you know if it's good or cautionary.

If you saw my list of calculators I have bookmarked. lol I use them religiously.

Those weren’t answering the safety question about the actual concentration tho.

This one will say if it's not safe to use. For example

I've wondered that before; if there's a point at which it's syrupy from too much mannitol in too little water. Then I thought about injecting myself with mannitol syrup, got grossed out, and just went and did something else.

Edit: Well this is concerning. I punched in the NAD+ reconstitution that I've always seen, which is 3 ml in a 500mg vial, and it has super high risk all over the place.
