I've taken 7 shots total. The last one was Tuesday 7.5. Yes I know I'm going up quickly but I'm not having any issues other than I've not lost a single pound. Plus my appetite is crazy. How long does it take to start working? I'm thinking about adding ret.

Everyone is different in how they respond. Titrating up doesn't guarantee you will lose quicker. If anything it may guarantee you some undesired sides, but you probably already know this 🙂 Is your appetite suppressed? Feeling fuller? I am a slower responder, took my time getting up to 15 then plateaued since November. I felt effects the first week as far as app and fullness, but the scale moved at a snail's pace, but I dropped 2 sizes in clothing. Have you noticed your clothes loosening?

Have you been on any GLPs before? Have you already lost weight before starting it? Age, weight , height?

Assuming the answer is no to the first two questions, it sounds like you are not super sensitive to GLP's, most people would have some appetite suppression or a bit of weight loss or some nausea by 7.5mg. I would suggest sticking with tirz for now, and then you will in the end know how well it works for you or not. If you start adding in reta or swapping to reta halfway through and you start responding you will not know if tirz works for you or not. Losing weight and keeping it off is a long game and Tirz is either the best drug available ( lowest side effects ) or the second best ( on weight loss ), and knowing for sure if it works for you or not is worth another month or 2. If you have zero side effects and do not have diabetes or other serious illnesses, you can increase doses a bit faster. There is always more risk of unpleasant side effects if you do that, but upping doses 2.5mg every 2 weeks with zero side effects is not especially dangerous. The tirz dose escalation protocol is fairly slow, compared to the others, and mostly that is a good idea.

And if you get to 15mg/w and still no effects then , maybe your tirz is dodgy or swap to reta and see if it works any better. But you will then know for sure if it is worth using for you, but if you mix it up half way through you will not know at the end. A picture I saw recently from one of the studies showed about 5 to 10% had minimal weight loss from full doses over a year so it is possible, and it would really be disappointing. Also double and triple check your dilution and dosing calculations, it is so easy to be out by a factor of 10.

If you have a different batch of tirz you can try , try it and see if it is the same, I have not heard of anyone on this forum getting vials with zero drug, but it can happen. If it was from a well known vendor who has been around a while it is less likely. A random vendor on tiktok maybe not so unlikely.

I was on ozempic for I think four years until they stopped the personal assistance program for people on Medicare. I was off a month before starting tirz. My starting weight on ozempic was 385 and end weight was 230. In the month my weight creeped back up to 240 and now I'm at 250. 😢 My sister passed away unexpectedly and I started stress eating. But I wasn't feeling much appetite suppression so it was easy to overeat. I started taking oz with an a1c of 72! With lots of discipline and oz I got it down to 64! I've been watching my blood sugars especially with the excellerated dosing. They are staying good. Not low at all.

I would Give it more time at least 4-8 weeks total (or until 2-4 shots at 7.5-10 mg) before calling it stalled. Track non-scale wins, measurements, clothes fit, energy, blood sugars ( already good ), less cravings over time. Keep calories/protein in check where possible despite appetite. If you want to try reta, start tiny, and track weekly. But honestly, since you're only at 7 shots and ramped fast to 7.5 with no issues, I'd ride tirz solo a bit longer ( push to 10 mg next if tolerated ) before layering, many hit their stride at 7.5-10 mg after the initial " nothing happening " phase.

Sorry about your sister. that shit's brutal, and stress eating is a normal response. You're already crushing the blood sugar side. Hang in there..

All of those explain a fair bit. For some reason people with diabetes, do not respond quite as well to the weight loss effects of GLP's, excellent for blood sugar control though. And the effects at start weight are very different to the effects starting 130 pounds down from there. And you probably have some tolerance to the gi side effects after only stopping ozempic a month ago. With that much weight loss hunger is normally higher and metabolic rate has dropped. And stress is always a trigger for putting weight back on, and my condolences for your sister.

It may be that using tirzepatide what you should be aiming for first and most importantly is to not put the 130 pounds back on. Given that tirz is generally better than ozempic for weight loss that should be possible, and hopefully a bit more. But in terms of long term health not putting all that weight back on is going to make an enormous difference, losing more is good and will help more, but previous research said losing 5 or 10% made a big difference to metabolic health.

I do not see any reason why higher doses of tirz wont work, especially given ozempic has already worked, and having been on ozempic, having high blood sugars at any time, and having lost a pile of weight all help to explain why 7.5 mg of tirz is not having a dramatic effect. I would not really change the advice about doses.

I know you've struggled for a bit here recently, but congrats on the big loss before it. I think there's a good chance you'll find success at higher doses of tirz. My mom was in a similar situation as you. She was on max dose victoza, followed by max dose ozempic for years before starting mounjaro. She said it felt like she wasn't taking anything until she got to 12.5 mg and ended up losing another 50lbs between 12.5 and 15 mg.

Sorry about your sister, you've been through a lot, the weight gain makes total sense in that kind of grief. As others have mentioned, that is a huge win with your A1C.

I don't know if this helps, but thought I'd throw it out here. My sister was on Oz for 2 yrs, switched to Tirz because there wasn't a lot of improvement with her A1C, when she reached 7.5 her A1C was improving, but the scale wasn't. She wasn't happy, but she stayed at 7.5 for 3 months (drs orders), by the 3rd month the scale started to move. Fast forward 4 months later, now at 12.5 and things are looking better everyday.

Not really.

Thank you everyone. I guess I've lost my patience. It took me 4 years to lose what I did. But it was a tool. I put the work in. I learned the amount of food to eat and took in my protein. I've learned a lot through my journey.

I'll give it more time and see what happens.

Thank you all for your condolences. It's been hard. She was my best friend!

Good news! It's working. I was just being impatient. I've lost 2 pounds! I haven't weighed myself for about a week or longer because I was discharged. I should know better! I've been through this all before. Thanks everyone for listening to me and offering advice. I will stay on the 7.5 until I see if this continues.

Now to get brave and buy grey. It will save me a lot!
