The results from the SURMONT-MAINTAIN study were published a few days ago:.

Tirzepatide for maintenance of bodyweight reduction in people with obesity in the USA (SURMOUNT-MAINTAIN): a multicentre, double-blind, randomised, placebo-controlled trial - PubMed

Eli Lilly.

pubmed.ncbi.nlm.nih.gov

It was a nicely designed study. All participants were on either on 10mg or 15mg (their maximum tolerated dose - MTD) up to week 60. Then at week 60 participants were assigned to one of three treatment groups. 1) continue on the MTD, 2) reduce to 5mg, or 3) placebo. For the next 24 weeks, they stayed on that treatment. At week 84, if the participant gained 50% or more of the weight loss, they were put back on the medication.

What we see isn't too surprising. Participants assignment to the MTD continuing treatment were able to maintain their weight loss (red line). Folks assigned to 5mg (blue line) regained at a lower rate the those on placebo (gray line). The gray line has greater weight loss than we would anticipate, but there are folks who were on 10mg and 15mg up to week 60 in that group. So that placebo group isn't placebo from day 1. If so, we know that that number would have been a lot less.

The Attain-Maintain trial studied orforglipron (Foundayo) for maintenance.

The blog The Incretin Space has a quick summary.

Interesting point there: " It’s almost all the slow responders/patients who didn’t lose much weight to begin with that need rescue therapy."
