Please suggest the next step

Hi All,

I am using Tirze since 11 weeks, I am currently at 7.5mg dose as for the last 2 injections, today I have to do my next injection, but in the last 2 weeks I had some significant side effects: nausea and constipation mainly, that I never had when I was on 5mg.

I have a box of reta (10mg), but I decided to not use until tirze was working.

Tirze is still working for weight reduction.

My dilemma is:

would it be better continue with 7.5mg tirze or step down to 5 mg Tirze and add 1mg of reta?

Please let me know your opinion

Why don't you try with 5,5mg or 6mg or 6,5 .....

Or you inject 5mg every 5 days?

Or two timed a week 3mg or 3,5mg??

There are so many possibilities!
 
i would go with Tirz until it no longer works. It's an amazing drug, and I would use it, if I didn't get depressed on high doses.

If side effects are bad, go down in dose, maybe try 6 mg ?

There’s no rule that you have to titrate up every 4 weeks - stay at the lowest dose that works for you. If tirz is working, I wouldn’t stack it with reta - just titrate up as needed.

the side effects I am facing are absolutely manageable, I was a little bit worried about the 7.5mg dosage because with the next injections the mean level in the body will increase at least for the next 2 injections.

If the side effects will increase as for the concentration level it could be a problem.

This is the reason of my dilemma.

What about the idea to tritade down to 5mg, and in case it will not work (increased appetite, food noise etc), I could add a little more on next sunday (3,5 day later)?

Docusate sodium for constipation as needed. Imho I would stick it out for a few weeks until doing any changes. Most of my early nausea was because I was still eating too much.
I was thinking that nausea has been caused by the type of food (much more vegetables and legumes) I was using in the last 2 weeks, more than the quantity. But I am still not convinced about that.

Yes. You can split your weekly dose in half and pin 3.5 days apart. That will help smooth out the peaks and dips. I had really bad GERD moving up from 5mg so I went to a split-dose schedule and eventually was able to move back to full dose every 7 days. It took my body 4-6 weeks to adjust but I had to play with my dosing a little.

I absolutely agree with you, but as it is something new for me, I am obsessed by any single small issue I am facing.

Thank you for the suggestion, it could be a valid choose in my case.

Thanks, yes those could be interesting options.
 
Another vote for sticking with Tirz. Reta... Just cuz ya got it doesn't mean you have to use it (yet). Most people looking to switch/stack do so because they feel like what they are taking isn't working and you are having the opposite problem. With the availability of these meds it's easy to think like we are back at the buffet loading our plates!

7.5mg is a 50% increase over 5mg. You have the flexibility to lower that potentially lessoning side effects and increasing stash longevity.

Update:

during the week I was on 6mg dose, the side effects were very similar to the ones when I was on 7.5mg, just a little bit smoother, so I decided to step down to 5mg, today is the 3rd day after the last injection and the side effects are very soft. So for the next steps I will go with 5mg until it works (last week I lost 1Kg).

This is the way. And when you decide to go back up, you might try 0.5mg increments. 5.5mg was my sweet spot for almost three months.

I can relate to wanting to understand everything that's happening. I'm reaearching to figure out my next step. Thank you for asking the questions you have. You help others in addition to yourself.
