Can someone explain food noise on Tirz?

I’m on my second week of Tirzepatide at 5mg and honestly can’t fully tell whether the “food noise” is gone or if part of this is placebo.

That said, I do think I’m responding pretty well so far.

I’m currently doing 16:8 intermittent fasting. Once I break my fast around noon, I get full on meals where I’d normally go back for seconds. There are also moments where I genuinely don’t feel like eating. However, I still get occasional cravings — especially for peanuts — and I can still eat a couple handfuls without any issue. I initially thought Tirz would completely eliminate cravings/food noise, so I’m curious whether others still experience some cravings while responding well overall.

A few observations so far:

Lost just over 2kg in the first week (not sure how much is water weight vs actual fat loss)

No major side effects

Had sulphur burps one day, but nowhere near as bad as some stories I’ve read, and they disappeared after sleeping

My resting heart rate on Fitbit seems slightly elevated compared to normal, though I haven’t received any abnormal HR notifications

Overall I feel pretty good, but I’m trying to understand what “normal” early response actually looks like. Curious to hear from others:

Did cravings completely disappear for you?

How long did it take before the appetite suppression/food noise reduction became obvious?

Did anyone else notice a slight HR increase early on?
 
For me it was a near constant noise in my head about what I would be cooking that day, what I might grab to eat while I was out, what food was in the kitchen, pantry, freezer. A noise that was ever present. In contrast, on Tirz I almost NEVER think about food, in fact have to actually MAKE myself think, "Have I eaten enough today?" With food noise I couldn't even be around food without it pulling me in, like if there's food out at an event, I'd gravitate to it and focus on it to exclusion of all else, it was first priority. Now...nah. I'll pass right by a box of donuts and not even notice. It no longer consumes me.

Yes, it's like the opposite of a craving.

As I've matured, I've become more and more sensitive to low blood sugar. Even before it indicates, I get shakey. But it's not like other food cravings. My doctor claimed to have cravings for fish. ? ? With gastric slow-down, one even gets food aversion after eating. For not being psychoactive, tirz is some interesting stuff. One could say it's psychoactive with one's gut biome, maybe?

Never, ever, EVER ask this. I lost my first thirty pounds on 2.2mg, I felt it fifteen minutes after my first shot. Was it placebo? I give now and gave then ZERO fucks. Hooray for the human brain, if it was.
 
I've always been a "grazer," preferring to eat small snacks throughout the day instead of eating more substantional meals. I'm on week 5 of tirz, and my constant urge to snack has been greatly reduced. I still get the occasional craving, but it's SO much easier to ignore. It feels like my mind is a lot quieter, which is a welcome change of pace.
 
For me, early on, the food noise disappeared quickly. Within 2-3 days, I realized that I was not thinking about food. Food related television commercials also stopped triggering my appetite.

totally agree. i still find myself wanting to snack but can be satisfied with something responsible, and i don't gorge myself at meals. the big difference for me (ymmv) was that i don't want to eat a pile of sweets at random times anymore and can resist some of those really unhealthy choices.

Thank you. This is super helpful.

I thought food noise getting quieter means no cravings and I have been struggling with why I still had cravings and was considering upping my dose.

Got this, then definitely have food noise reduced. I have a sweet tooth and was offered a box of artisanal chocolate last week and I found myself passing on it.

I NEVER SAY NO TO CHOCOLATE.
 
I am still getting hungry at times. And even eating some large meals. But the total amount of food throughout the day is less and the incessant thinking about food is much, much less. What a relief.
 
For me it meant that I could think about the foods I like but not HAVE to have them. That 9 of the 10 pack of peanut butter cups my MIL gave me for Easter are still in the cupboard. It did take me awhile in the beginning to separate that "mouth hunger" from real hunger. I spent a lot of time "checking" to see if I still wanted snacks or treats and feeling like I did...until I eventually started to notice that if I was busy doing other things and not putting my focus on food, then it wasn't bothering me. And that it took way less to satisfy any cravings (5 fries, 5 chips, 1-2 pieces of chocolate) when I did decide to give in. I no longer thought of something and then had to have ALL of it as soon as possible.

I'm 9 months in now and the suppression has lessened for me but Tirz still lets me be the boss of my cravings rather than the other way around.
 
Cravings unfortunately did not go away for me, but the noise was fading after maybe the 2nd week. The only thing that kills a craving for me is if I get nausea from my pin that week. In which case I want nothing at all.
 
Food noise to me is going to the grocery store and automatically buying the problematic foods I used to eat. Now when I go to the grocery store, I don’t even think twice about buying those processed foods anymore. I do notice the cravings start to come back when I’m on the last day before my shot or if I skip a week or two.
 
For me, the constant cravings that resulted in snacking 'just because' went away. I used to be able to snack like 2-3 Popcorn, then some other salty and savoury stuff just because I was bored. Now I don't even think about those stuff. Don't really search for them, even when they're in my pantry. same as with the user above, the day before my shot I can feel the noise starting to come back. It really is a game changer for me.

I have started using mounjaro 7 months ago at 2.5mg under doctor's prescription. Felt hunger noise suppression within first 24 hours. After a week hunger noise was completely suppressed. Rarely thought that I needed some cookies with sugar when I saw them in a snacks automate. Bought one. But could not eat completely. One month ago tried reta and after 2 days hunger noise was back. I could easily ignore it. But it was there. In my opinion tirz is beter than reta for hunger suppression.
 
Also on my second week. Tirz 2.5mg.

I was completely at the mercy of food noise before. Every 2-3 hours I would start to feel a need to eat, I would get brain fog and become incredibly hangry if I didn't get something to eat within an hour or so. It was difficult to manage and I don't doubt it made me difficult to be around!

Now it's like night and day. I'm not a slave to food anymore. I'm eating, but it's not hard to maintain a small deficit. My meals are smaller and I'm not loitering around the kitchen looking for something to satisfy my insatiable hunger.

On day 6 this week however the hanger did return! When I didn't eat within a decent time frame (I was waiting for someone to go for lunch) all the usual feelings of brain fog and frustration returned. I need to factor this in next week and not forget that it can still happen!
 
5mg per week is the minimum therapeutic dose. If you're doing well and happy at that level, great!

But if you're not getting all the expected/desired effects, or the "honeymoon" effect wanes, it's completely normal and appropriate to go up by 2.5mg increments, four weeks at each step, until you hit 15mg per week.

The four-week steps are to allow you to identify undesired side effects while they're minimal vs feeling like a safe just dropped on your digestive tract.

If, when you continue increasing dosage per standard protocol, you find the Day 6 effect still happening, it means you're a good candidate for 5-day or 3.5 day injection spacing. With adjusted dosage each time, of course.

I took my first shot of tirzepatide at about 11am and by the early afternoon that same day I felt the effects. But unlike a lot of folks who suffer with mental cravings or constant thoughts about food, I had something different. I was basically a grazer all day long. And I would do this mindlessly. I'd just be standing at the fridge looking for something to eat at random times. All day long. I also would snack a lot. That first afternoon, no more fridge surfing. No more finding myself mindlessly in front of the fridge and if I had a snack I did it intentionally. That's basically lasted for the last 17 months being on tirzepatide. I also did a bit of CBT and meet with a nutritionist later on in my journey. But it was definitely the tirzepatide.
